LIVING WELL CLASSES
TO BE HELD AT GRASONVILLE SENIOR CENTER AND SUDLERSVILLE SENIOR CENTER
The Living Well program “Take Charge of your Health” a replication of Stanford University’s highly successful Chronic Disease Self Management Program, is taught by Lay Leaders, who themselves suffer from chronic diseases such as arthritis, high blood pressure, obesity, and/or diabetes. The participants meet once a week for 6 weeks, will learn easy and effective strategies in several areas, including nutrition, physical activity, medication management and communication. They will develop personal action plans and use the tools discussed to achieve goals that positively impact their health, feeling better in the process.
